The temporomandibular joint (TMJ) is the hinge just in front of your ear. You use it every time you speak, chew or swallow. When it is not working properly, the symptoms are easy to mistake for something else: headaches around the temples, earache with nothing wrong in the ear, a jaw that clicks or catches, or pain when you chew.

A common cause is grinding or clenching the teeth at night, which most people do not know they do. It shows up on the teeth as flat, worn edges long before anyone notices the habit. Once the grinding is under control, teeth that have worn down badly can be rebuilt with crowns.

Most TMJ problems get better with simple, non-surgical treatment — a night guard, bite adjustment and jaw exercises. Surgery is a last resort and rarely needed. Because a maxillofacial surgeon checks your jaw, you get a proper diagnosis of what kind of problem you have, instead of a guard fitted as a guess.

Where the joint pain comes from a bite that does not meet properly, correcting the bite treats the cause, not just the symptom. Sometimes that means braces. Occasionally, in more severe structural cases, it means jaw surgery. Most people never need either, and a night guard plus a few habit changes is the whole treatment.

TMJ treatment: what is involved

  1. 1

    Assessment

We examine the joint and the muscles, check how far your jaw opens, and find where the pain actually starts.

  2. 2

    Imaging

X-rays or a scan, where we need to see the structure of the joint itself.

  3. 3

    Non-surgical treatment

A night guard made for your teeth, jaw exercises, and bite adjustment where the teeth are causing the problem.

  4. 4

    Review

Most cases improve within weeks. If the problem continues, we assess it again instead of leaving it.

When jaw pain is an emergency

Jaw pain is usually the joint, the muscles or a tooth. But pain in the jaw, especially the left side, can also be a sign of a heart attack, particularly in women, older people and people with diabetes. Do not wait for a dental appointment if jaw pain comes with any of the signs below. Call 108 or 112, or go to the nearest hospital emergency department.

  • Chest pain, pressure or tightness, or pain spreading to the arm, neck or back
  • Breathlessness, sweating, nausea or feeling faint
  • Jaw pain that comes on with exertion and eases with rest
  • A jaw that is stuck open after a yawn or injury, or facial swelling with fever or difficulty swallowing: same-day care

What helps jaw pain at home

Most jaw pain from the joint and muscles settles with rest and simple care. The US National Institute of Dental and Craniofacial Research notes that for many people TMJ symptoms are temporary and improve without invasive treatment.

  • Soft food for a week: dal, khichdi, curd, cut sabzi. Avoid chewing gum, hard nuts, chikki and very large bites.
  • A warm compress on the side of the face for 10 to 15 minutes, a few times a day.
  • Notice clenching during the day: lips together, teeth apart, jaw relaxed.
  • Avoid wide yawning and resting the chin on your hand.
  • If pain lasts more than two weeks, the jaw clicks and locks, or it keeps coming back, have it examined.

TMJ treatment in Jodhpur — common questions

Why does my jaw hurt? (Jabde me dard kyu hota hai?)

Jaw pain most often comes from the jaw joint (TMJ) and the chewing muscles, usually because of teeth grinding or clenching, stress, or a bite that does not meet evenly. A decayed or infected tooth, a wisdom tooth or sinus trouble can also cause it. Jaw pain with chest pain or breathlessness needs emergency care.

How is jaw pain treated? (Jabde me dard ka ilaj)

Most jaw pain from the TMJ is treated without surgery: a custom night guard, soft food for a while, warm compresses, jaw exercises and, where the teeth are the cause, bite adjustment. At Pain-Off Dental Care in Jodhpur, an MDS oral and maxillofacial surgeon first finds the cause, so treatment is not a guess.

Can jaw pain be a sign of a heart attack?

Yes, pain in the jaw, often on the left side, can be a symptom of a heart attack, especially with chest pain, breathlessness, sweating or nausea. It is more common in women, older adults and people with diabetes. If jaw pain comes with any of these, call 108 or 112 and go to a hospital immediately.

Can TMJ problems go away on their own?

Many TMJ problems improve on their own or with simple self-care within a few weeks. The US National Institute of Dental and Craniofacial Research advises starting with conservative treatment. Pain that lasts more than two weeks, a jaw that locks, or pain that keeps returning should be examined, as grinding can wear the teeth down.

Is my headache related to my jaw?

A headache can come from the jaw. Teeth grinding and clenching overwork the muscles at the temples, causing a headache that is often worst on waking and centred on the temples. If you also have a tight jaw in the morning or teeth worn flat, a TMJ check is worth doing.

Can TMJ cause ear pain or ringing in the ears?

Yes, the jaw joint sits just in front of the ear, so TMJ problems often cause earache, a feeling of fullness or ringing in the ear, even when the ear itself is healthy. If an ear specialist has found nothing wrong and the jaw clicks or aches, the TMJ is a likely cause.

Why do I grind my teeth at night? (Daant peesna kyu hota hai?)

Teeth grinding at night, called bruxism, is linked to stress, anxiety, poor sleep, snoring and sleep apnoea, and sometimes to caffeine, alcohol or certain medicines. Most people do not know they do it until a family member hears it or a dentist sees flat, worn teeth. A night guard protects the teeth while the cause is addressed.

Is a clicking jaw serious?

A clicking jaw without pain is common and usually needs no treatment. It is worth having checked if the click comes with pain, the jaw catches or locks, or you cannot open your mouth fully. These signs suggest the disc inside the joint is affected and treatment helps.

Will a night guard stop me grinding my teeth?

A night guard does not stop the grinding habit, but it takes the force off the teeth and the jaw joint, so the guard wears down instead of your teeth. A custom-made guard from a dentist fits closely and is far more comfortable than a shop-bought one, so people actually keep wearing it.

Will I need surgery for TMJ?

Surgery for TMJ is very rarely needed. Most jaw joint problems settle with a night guard, self-care and exercises. Procedures such as arthrocentesis, which washes out the joint, are kept for joints that lock or stay painful despite months of other treatment. Dr Suveet Mathur, a maxillofacial surgeon, assesses whether that applies.
