Wisdom teeth come in last, and often there is no room left for them. A tooth that cannot come through fully is called impacted. It may lie sideways, stay partly buried, or press on the tooth in front. That causes repeated infections, swelling, a stiff jaw and decay on the healthy molar next to it.

Removing an impacted wisdom tooth is surgery, not a simple extraction. The tooth may need to be cut into sections and removed in pieces. The lower ones sit close to the nerve that gives feeling to the lip and chin. Checking where that nerve lies beforehand is what separates a routine recovery from a complication.

This is a core part of Dr Suveet's surgical work, and one of the procedures other Jodhpur clinics refer to him.

Pain from a wisdom tooth is often not the tooth at all, but the gum over it. That gum traps food and gets infected, because a toothbrush cannot reach behind it. Sometimes treating the gum settles it and the tooth can stay. When it cannot, this is proper oral surgery rather than a simple pull, and it is worth having it done by someone who does it every week.

It is also worth checking for the real culprit. Pain at the very back of the mouth is sometimes not the wisdom tooth but the molar in front of it. That molar decays easily because it is just as hard to reach — and it needs a root canal, not an extraction. The X-ray shows which of the two it is before anything is removed.

Wisdom tooth removal: what is involved

  1. 1

    X-ray assessment

We map where the tooth sits and, most importantly, how close it is to the nerve canal.

  2. 2

    Anaesthetic

Local anaesthetic. The area is fully numb before anything begins, so you feel pressure, not pain.

  3. 3

    Surgical removal

An opening is made, the tooth is cut into sections if needed, and it is removed piece by piece. This is gentler on the bone than forcing it out whole.

  4. 4

    Closure

Stitches where needed, usually the dissolving kind, with written aftercare instructions.

Akal daad: when it can stay, and when it should come out

Not every wisdom tooth needs removing. The UK's National Institute for Health and Care Excellence (NICE) advises against removing healthy, symptom-free wisdom teeth just in case. The decision depends on what the tooth is doing, which the X-ray shows.

  • Usually fine to keep: fully through the gum, upright, biting against the tooth above, and easy to clean.
  • Usually better out: repeated infection or swelling of the gum over it (pericoronitis), decay in it or in the molar in front, a cyst, or damage to the root of the next tooth.
  • Watch with X-rays: impacted but causing no problems. Reviewed at check-ups rather than removed straight away.
  • Upper wisdom teeth are usually simpler to remove than lower ones, which sit closer to the nerve.

After wisdom tooth removal: the first week

Most of the discomfort is in the first two to three days. Following the aftercare protects the blood clot in the socket, which is what heals the area.

  • Bite on the gauze for 30 to 45 minutes. Some oozing for the rest of the day is normal.
  • Ice on the outside of the cheek, 15 minutes on and off, for the first day.
  • Soft, lukewarm food: khichdi, dal, curd, upma, mashed potato, soft idli. Nothing crunchy, spicy or very hot for a few days.
  • No smoking, gutkha, straws or spitting hard for at least 3 days. These dislodge the clot.
  • From the next day, gentle warm salt-water rinses after meals.
  • Call the clinic if pain is getting worse on day 3 or 4, bleeding will not stop, or you have a fever or spreading swelling.

Wisdom tooth removal in Jodhpur — common questions

When do wisdom teeth come in? (Akal daad kab aati hai?)

Wisdom teeth, called akal daad, usually come in between the ages of 17 and 25, behind the last molars. Most adults have four, one in each corner, but some people have fewer or none. Many wisdom teeth never come through fully because there is not enough room in the jaw, and these are called impacted.

Why does my wisdom tooth hurt, and what should I do? (Akal daad me dard ho to kya karein?)

Wisdom tooth pain is most often caused by infection of the gum flap over a partly erupted tooth, where food gets trapped. Warm salt-water rinses and an ordinary painkiller taken as directed help in the short term. See a dentist within a few days, and the same day if you have swelling, fever or difficulty opening your mouth.

Is it necessary to remove a wisdom tooth? (Akal daad nikalna zaroori hai?)

Removing a wisdom tooth is not always necessary. A tooth that is fully through, upright and easy to clean can stay. Removal is advised when a wisdom tooth causes repeated infection, decay, damage to the molar in front, or a cyst. The UK's NICE guidance advises against removing healthy, symptom-free wisdom teeth as a precaution.

What is an impacted wisdom tooth? (Akal daad tedi ho to?)

An impacted wisdom tooth is one that cannot come through the gum fully, because it is tilted, lying sideways or blocked by the tooth in front. Impacted lower wisdom teeth are the most common cause of repeated swelling at the back of the mouth. Removing one is a surgical procedure, best done by an oral and maxillofacial surgeon.

How much does wisdom tooth removal cost in Jodhpur?

The cost of wisdom tooth removal in Jodhpur depends mainly on how the tooth sits: a fully erupted upper wisdom tooth is a simple extraction, while an impacted lower one lying sideways is surgery. Pain-Off Dental Care in Sardarpura gives you the cost after the X-ray, before the procedure, so you know which kind yours is.

Is wisdom tooth removal painful?

Wisdom tooth removal is done under local anaesthetic, so you feel pressure and movement but not pain. Afterwards, soreness and swelling peak around 48 hours and settle over about a week, managed with ordinary painkillers. At Pain-Off Dental Care, impacted teeth are removed by Dr Suveet Mathur, an MDS oral and maxillofacial surgeon.

How long does recovery take after wisdom tooth removal?

Recovery after wisdom tooth removal usually takes about a week. Swelling is worst around 48 hours, and most people take one to two days off work or college. Soft food is easiest for the first few days. The gum closes over in two to three weeks, and the bone underneath fills in over a few months.

What should I eat after wisdom tooth removal? (Akal daad nikalne ke baad kya khaayein?)

After wisdom tooth removal, eat soft, lukewarm food for the first few days: khichdi, dal, curd, upma, soft idli and mashed potato. Avoid hot, spicy, crunchy and sticky food, and do not use a straw, smoke or chew gutkha for at least three days, as these can dislodge the clot and cause a dry socket.

What is a dry socket?

A dry socket happens when the blood clot in a tooth socket is lost too early, leaving the bone exposed. It causes throbbing pain that starts two to four days after the extraction, often spreading to the ear, and a bad taste. Smoking and using straws raise the risk. A dentist can dress the socket, which relieves the pain quickly.

Can wisdom tooth removal damage the nerve?

A nerve that gives feeling to the lower lip and chin runs close to the lower wisdom teeth. Temporary numbness after removal happens in a small percentage of cases and usually recovers; permanent numbness is rare. An X-ray, or a 3D scan when the tooth is very close to the nerve, shows the risk before surgery.
